The following questionnaire was developed by a team of researchers with the University of Manitoba and the Children's Hospital Research Institute of Manitoba. We are looking to identify and describe health programs across Canada that are available for children and youth living with obesity and their families. The goal is to update the nation-wide survey on health services that was done 5 years ago. The original study can be found here. From this survey we aim to create an online list of services available to children and youth with obesity in order to showcase the important elements of each program and where it is offered.
This survey will take about 20 minutes to complete. You will be able to save and exit the survey if you need to (scroll to the bottom of the page). This will allow you to continue where you left off at a later time (make sure you save your code). If you would rather have a paper copy, or do the survey by phone, please call the number below. The survey will be available to complete for a few months. We encourage you to share it with others who you think would be interested in completing it.
We hope this study will benefit the children and youth of Canada by helping families and healthcare providers find healthy lifestyle programs more easily. We do not see any risks in completing our survey.
The information related to the description of the programs is the sole responsibility of the contact person of the program. Please make sure that you fill in your contact information, as we may need to contact you. The Canadian Obesity Network and the researchers reserve the right not to post the description of certain centres.
